Once a participant has successfully completed all 4 courses listed below, they are eligible to apply for AAPACE’s Post-Acute Care Certification program. Upon achieving Post-Acute Care Certification through AAPACE, the applicant will be authorized to use the credentials PAC-C (Post-Acute Care Certified) after their name. Post-Acute Care Course
This course is a necessary review for all nurses that work with the adult and geriatric population experiencing multi- or complex health conditions, particularly in the post-acute care and long-term care setting. In this course, the nurse will review common and critical medical diagnoses and treatment guidelines often seen with this patient population, as well as physical assessment techniques, management of alterations in health, high-risk medications, how to prevent unnecessary re-hospitalizations, patient teaching principles, cultural considerations for patients and how to effectively coordinate care with the nursing assistant. Principles of effective communication, documentation, and collaboration are continuous throughout the course.
Participants will be granted 4 continuing education units (CEU’s), which includes 1 hour of pharmacology, and a certificate upon successful completion. Brought to you in collaboration with the Illinois College of Nursing.
Wound Care Course
Topics covered in this course include: Wound prevention, principles of wound care, wound treatment guidelines and nursing care management. Pressure, diabetic, venous and arterial ulcers, as well as other skin disorders (edema, burns, skin tears, etc.) will be covered. The course reviews physical assessment techniques, management of alterations in health, high-risk treatments and medications, prevention of unnecessary re-hospitalizations, patient teaching principles, cultural considerations and effective coordination of care with the nursing assistant. Principles of effective communication, documentation, and collaboration are continuous throughout the course.

Dementia & Alzheimer’s Course
This course includes a review of the diagnosis, prognosis, treatment and nursing care for Alzheimer’s disease and dementia. In this course we will review assessment techniques to identify and understand common behaviors associated with dementia and Alzheimer’s disease, learn how to provide patient-centered care, assist with Activities of Daily Living and effectively communicate while still promoting patient autonomy and nurturing the nurse-patient relationship. Principles of effective communication, documentation, and collaboration are continuous throughout the course.

Leadership & Innovation Course
Be the change agent that leads your organization to a progressive culture of cohesiveness and growth mindset. This course is intended to help one become familiar with the different leadership styles, identify own strengths and areas for growth as well as develop an innovative growth mindset to be an effective, efficient and inspirational nurse leader in any healthcare setting. Effective communication, delegation, peer evaluation and conflict resolution techniques are discussed. The nursing process is reviewed and applied to real life examples on how one can enhance their critical thinking skills and identify ways to turn an everyday workplace challenge into an opportunity for growth and innovation.

Post-Acute Care Certification (PAC-C)
Once you have successfully completed all 4 courses, you are eligible to apply for AAPACE’s Post-Acute Care Certification program. Upon achieving Post-Acute Care Certification through AAPACE, you will be authorized to use the credentials PAC-C (Post-Acute Care Certified) after your name. These credentials are to be placed after your highest degree earned. For example, for a Licensed Practical Nurse whose name is Jennifer Smith, she would write her name and credentials as follows:

Certification is optional. However, once certified, you will need to retake the required courses and re-submit your certification registration application every 2 years if you wish to continue to be post-acute care certified. To renew your 2-year certification, you must complete each course again as the content is routinely updated to adhere to current healthcare standards. All courses must be completed within a 6-month period. Once each of the four courses is completed again, you can apply to have your certification renewed for another 2-year window.
If you choose not to pursue certification, you will still receive continuing education credit for the courses you have completed, but you will not be authorized to use the credentials PAC-C.
